แพ็กเกจตรวจสุขภาพหัวใจ (Heart Checkup Packages)

หมายเหตุ 

• กรุณางดน้ำและอาหารอย่างน้อย 8-12 ชั่วโมง

• ทุกโปรแกรมรวมค่าแพทย์แจ้งผลตรวจสุขภาพและค่าบริการ รพ.แล้ว

• ไม่สามารถเปลี่ยนแปลงรายการตรวจในแพคเกจได้

• โรงพยาบาลขอสงวนสิทธิ์ในการเปลี่ยนแปลงรายการตรวจโดยไม่แจ้งล่วงหน้า

• กรุณาสอบถามรายละเอียดและนัดหมายล่วงหน้าที่ ศูนย์หัวใจ โทร.0 3825 999 เวลา 8.00 -16.00 น.

• กรุณามาก่อนเวลานัดอย่างน้อย 30 นาที

• option หมายถึง รายการที่แนะนำตรวจเพิ่มในราคาพิเศษ

• ราคาแพ็กเกจไม่รวมค่าแพทย์เฉพาะทางและค่ายาทั้งหมดที่เกิดขึ้นระหว่างเข้ารับการตรวจ (ถ้ามี)

Remark

• You are advised not to eat or drink (other than plain water) for 8-12 hours before the examination.

• All packages are inclusive of doctor fee to inform health examination report and hospital service charge.

• The items in the package cannot be changed.

• The hosptial reserves the right to change the package without prior notice.

• To make an appointment in advance, please call 0 3825 9999.

• Please arrive at the hospital at least 30 minutes before the appointment time

• Option means recommended and additional tests that are in special price.

• The package price does not include specialist fees and any medication costs incurred during the examination (if applicable).

ขั้นตอนการเข้ารับบริการ

1. เมื่อดำเนินการชำระค่าบริการของสินค้าและโปรโมชั่นต่างๆสำเร็จแล้ว ภายใน 4 ชั่วโมง

ผู้รับบริการจะได้รับ E-coupon สำหรับแสดงในวันที่เข้ารับบริการ

2. กรุณาทำนัด และเลื่อนนัดหมายล่วงหน้า อย่างน้อย 1 วัน ก่อนเข้ารับบริการ โดยเงื่อนไขเป็นไปตามที่โรงพยาบาลกำหนด

How to use E-coupon 

1. E-coupon will be delivered via LINE chat. Please check for accuracy within 4 hrs.

(If there is an error information, please contact Bangkok Hospital Pattaya Official Shop via LINE chat)

2. To use E-coupon for health treatments or services at Bangkok Hospital Pattaya,

 E-coupon can be presented to registration counter in digital format or print out on the appointment date.
