เงื่อนไขการรับบริการ

1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำ E-Coupon มาแสดงเท่านั้น

2. สำหรับใช้บริการผู้ป่วยนอกเท่านั้น / อัตราดังกล่าวรวมค่าแพทย์ผู้ป่วยนอกแล้ว

3. E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้

4. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้

5. กรุณาโทรนัดหมายล่วงหน้าก่อนรับบริการอย่างน้อย 1 วัน ที่ Contact Center โทร 02-310-3000 หรือ 1719

6. สามารถรับบริการที่ คลินิกวัคซีน ชั้น 1 อาคารโรงพยาบาลกรุงเทพ (โซน D) ซ.ศูนย์วิจัย โทร. 02-310-3003

เปิดบริการทุกวัน เวลา 08:00 - 16:00 น. 

7. สำหรับนักเรียน นักศึกษาที่จะเดินทางไปศึกษาต่อต่างประเทศ รับบริการได้ที่ ศูนย์บริการผู้ป่วยต่างประเทศ (IMS) ชั้น 4 อาคาร R โรงพยาบาลกรุงเทพ

เปิดบริการทุกวันจันทร์ – ศุกร์ เวลา 07.00 – 16.00 น. และวันเสาร์ – อาทิตย์ เวลา 07.00 – 15.00 น. โทร 02-310 3256, 02 755 1256

8. แพ็กเกจนี้ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

วันหมดอายุ E-Coupon

1. E-Coupon มีอายุการใช้งานถึงวันที่ 31 ธันวาคม พ.ศ. 2569

2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้

3. แพ็กเกจสุขภาพราคานี้ สามารถสั่งซื้อได้ถึง 31 ธันวาคม พ.ศ. 2569 เท่านั้น

หมายเหตุ

1. การยกเลิกคำสั่งซื้อที่ไม่เกี่ยวข้องกับเหตุผลทางการแพทย์ โรงพยาบาลขอสงวนสิทธิ์ในการคืนเงินหรือยกเลิกสินค้า

2. ยอดเงินที่ระบุในใบเสร็จเป็นยอดชำระจริง ไม่รวมส่วนลดจากโปรโมชั่นใด ๆ

3. ใบเสร็จรับเงินจะได้รับในรูปแบบออนไลน์เท่านั้น หากต้องการใบเสร็จตัวจริงสามารถติดต่อเจ้าหน้าที่การเงินในวันที่เข้ารับบริการ

1. Service is reserved exclusively for customers who present the E-Coupon.

2. Without the coupon, the regular package price will apply.

3. This E-Coupon cannot be exchanged for cash.

4. E-Coupon is only applied to specific items as mentioned and changes cannot be made.

5. Valid for outpatient services only. The price includes physician fees.

6. This package does not include additional treatment or medication costs as prescribed by a physician.

7. Please make an appointment at least 1 day in advance via Contact Center at +66 2 310 3000 or 1719.

8. Services are available at the Vaccine Clinic, 1st Floor, Bangkok Hospital (Zone D), Soi Soonvijai. Tel. +66 2 310 3003

Open daily from 08:00 AM – 04:00 PM.

9. For students traveling abroad for study, services are available at the International Medical Services (IMS), 4th Floor, Building R, Bangkok Hospital.

Open Monday – Friday: 07:00 AM – 04:00 PM

Saturday – Sunday: 07:00 AM – 03:00 PM

Expiration Date of E-Coupon:

1. The E-Coupon is valid until December 31, 2026.

2. If the E-Coupon expires, the hospital reserves the right to deny service, and the E-Coupon cannot be extended or refunded.

3. This health package is available for purchase until 31 December 2026.

Note:

1. In case of cancellation request for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund.

2. The receipt amount displayed is the actual payment received, excluding any promotional discount.

3. An e-receipt will be provided. A physical receipt can be requested at the cashier counter on the day of service.
