เงื่อนไขการรับบริการ

1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำคูปองมาแสดงเท่านั้น

2. คูปองนี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้

3. คูปองนี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้

4. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น

5. ชุดตรวจสุขภาพ ไม่รวม ค่าแพทย์กรณีเจาะจงแพทย์เฉพาะทางในการตรวจสุขภาพและการตรวจทางรังสี, ค่ายา หรือค่ารักษาเพิ่มเติมที่แพทย์สั่ง

6. กรุณานัดหมายล่วงหน้า อย่างน้อย 3 วัน ได้ที่ Contact Center โทร. 1719

7. สามารถรับบริการได้ที่ โรงพยาบาลกรุงเทพ ชั้น 2 อาคาร D

8. ราคาดังกล่าวรวมค่ารังสีแพทย์ และอายุรแพทย์เฉพาะทางด้านหัวใจ

วันหมดอายุ E-Coupon

1. E-Coupon มีอายุการใช้งานภายในวันที่ 31 ธันวาคม พ.ศ. 2569

2.หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้

3.ชุดตรวจสุขภาพราคานี้ สามารถสั่งซื้อได้ถึง 31 ธันวาคม พ.ศ. 2569 เท่านั้น

หมายเหตุ

1. การยกเลิกคำสั่งซื้อที่ไม่เกี่ยวข้องกับเหตุผลทางการแพทย์ โรงพยาบาลขอสงวนสิทธิ์ในการคืนเงินหรือยกเลิกสินค้า

2. ยอดเงินที่ระบุในใบเสร็จเป็นยอดชำระจริง ไม่รวมส่วนลดจากโปรโมชั่นใด ๆ

3. ใบเสร็จรับเงินจะได้รับในรูปแบบออนไลน์เท่านั้น หากต้องการใบเสร็จตัวจริงสามารถติดต่อเจ้าหน้าที่การเงินในวันที่เข้ารับบริการ

⏬️ Service Terms and Conditions⏬️

1. Service is reserved exclusively for customers who present a valid E-Coupon.

2. This E-Coupon cannot be exchanged for cash.

3. The E-Coupon is applicable only to the specified items, and no changes can be made.

4. These packages are for outpatients who are physically independent.

5. These packages exclude fees for additional specialists, as well as any medications and treatments prescribed by the doctor.

6. Please make an appointment at least 3 days in advance by contacting the Contact Center at 1719.

7. Services are available at Bangkok Hospital, 2nd floor, Building D.

8. This price includes medical imaging costs and the cardiologist’s consultation fee.

Expiration Date of E-Coupon:

1. The E-Coupon is valid until December 31, 2026.

2. If the E-Coupon expires, the hospital reserves the right to deny service, and the E-Coupon cannot be extended or refunded.

3.This health check package can be ordered until December 31, 2026.

Note:

1. In case of cancellation request for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund.

2. The receipt amount displayed is the actual payment received, excluding any promotional discount.

3. An e-receipt will be provided. A physical receipt can be requested at the cashier counter on the day of service.
