เงื่อนไขการรับบริการ

1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำคูปองมาแสดงเท่านั้น

2. ในกรณีที่ไม่นำคูปองมาแสดง ท่านต้องชำระค่าใช้จ่ายตามราคาชุดตรวจ

3. คูปองนี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้

4. คูปองนี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้

5. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น

6. แพ็กเกจนี้ไม่รวม ค่าแพทย์ในกรณีเจาะจงแพทย์

7. แพ็กเกจนี้ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

8. กรุณาทำนัดล่วงหน้าก่อนเข้ารับบริการอย่างน้อย 3 วัน

10. สามารถรับบริการได้ที่ ศูนย์กระดูกและข้อ ชั้น 1 รพ.กรุงเทพอินเตอร์เนชั่นแนล ที่ศูนย์กระดูกและข้อ ชั้น 1 โรงพยาบาลกรุงเทพอินเตอร์เนชั่นแนล

วันอังคาร เวลา 09.00 – 15.00 น.

วันพุธ เวลา 9.00 – 20.00 น.

วันพฤหัส เวลา 17.00-20.00 น.

วันศุกร์ 17.00 – 20.00 น.

วันอาทิตย์ 9.30-12.00 น.

11. ราคานี้สำหรับคนไทยและ Expat เท่านั้น

วันหมดอายุ E-Coupon 

1. E-Coupon มีอายุการใช้งานภายในวันที่ 31 มกราคม พ.ศ. 2569

2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้

3. ชุดตรวจสุขภาพราคานี้ สามารถสั่งซื้อได้ถึง 31 มกราคม พ.ศ. 2569 เท่านั้น

หมายเหตุ

1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อโดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ

2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

1. Service is reserved for customers who present the coupon upon arrival.

2. In case the coupon is not presented, the customer must pay the examination fee as stated.

3. The coupon cannot be exchanged for cash.

4. The coupon is valid only for the specified examination and cannot be altered.

5. This package is for outpatients who are physically dependent.

6. This package excludes doctor fees in case additional specialists are required.

7. This package does not cover additional treatment or medication prescribed by the doctor.

8. Please make an appointment at least 3 days in advance.

10.Services can be received at Orthopedic Center, 1st floor Bangkok International Hospital.Tuesday: 9.00 a.m. – 03.00 p.m.

Wednesday: 09.00 a.m. – 08.00 p.m.

Thursday: 05.00 p.m. - 08.00 p.m.

Friday: 05.00 p.m. - 08.00 p.m.

Sunday: 09.30 a.m. – 12.00 a.m.

at Spine Center, 2nd floor, Bangkok International Hospital 

Saturday: 01.00 p.m. – 05.00 p.m.

11. The price is applicable to both Thai nationals and expatriates.

Expiration Date of E-Coupon:

1. The E-Coupon is valid until January 31, 2026.

2. If the E-Coupon expires, the hospital reserves the right to deny service access, and the E-Coupon cannot be extended or refunded.

3.This health check package can be ordered until January 31, 2026.

Note:

1. In case customers request cancellation of the order without valid reasons from the hospital, the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.

2. The receipt amount displayed is the actual payment received, excluding any promotional discount.
