เงื่อนไขการรับบริการ

1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำคูปองมาแสดงเท่านั้น

2. คูปองนี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้

3. คูปองนี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้

4. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น

5. แพ็กเกจนี้ ไม่รวม ค่าแพทย์ในกรณีเจาะจงแพทย์เฉพาะทางในการตรวจสุขภาพและตรวจทางรังสี

6. แพ็กเกจนี้ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

7. กรุณานัดหมายล่วงหน้า อย่างน้อย 3 วัน

9. สามารถรับบริการได้ที่ โรงพยาบาลหัวใจกรุงเทพ (ซอยศูนย์วิจัย) ชั้น 1 อาคาร H

10. ราคาดังกล่าวรวมค่ารังสีแพทย์ และอายุรแพทย์เฉพาะทางด้านหัวใจ

11.ราคานี้สำหรับคนไทยและ Expat เท่านั้น

วันหมดอายุ E-Coupon

1. E-Coupon มีอายุการใช้งานภายในวันที่ 31 มกราคม พ.ศ. 2569

2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้

3. ชุดตรวจสุขภาพราคานี้ สามารถสั่งซื้อได้ถึง 31 มกราคม พ.ศ. 2569 เท่านั้น

หมายเหตุ

1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อโดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ

2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

1. Service is reserved exclusively for customers who present the E-Coupon.

2. This E-Coupon cannot be exchanged for cash.

3. E-Coupon is only applied to specific items as mentioned and changes cannot be made.

4. These packages are for outpatients who are physically dependent. 

5. This package excludes doctor fees in case additional specialists are required.

6. This package excludes treatment fees or medications prescribed by the doctors, if any. 

7. Please make an appointment at least 3 days in advance.

8. For more information or to make an appointment, please contact Bangkok Heart Hospital (Soi Soonvijai) at 02-755-1371, 02-310-3370, or the Contact Center at 1719.

9. Services are available at Bangkok Heart Hospital (Soi Soonvijai), 1st Floor, Building H.

10.This price includes the cost of medical imaging and cardiologist consultation fee.

11.This price is for Thai citizens and expatriates only.

Expiration Date of E-Coupon:

1. The E-Coupon is valid until January 31, 2026.

2. If the E-Coupon expires, the hospital reserves the right to deny service access, and the E-Coupon cannot be extended or refunded.

3.This health check package can be ordered until January 31, 2026.

Note:

1. In case customers request cancellation of the order without valid reasons from the hospital, the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.

2. The receipt amount displayed is the actual payment received, excluding any promotional discounts.
