จอประสาทตาเสื่อม เป็นโรคที่เกิดจากความเสื่อมบริเวณส่วนกลางของจอประสาทตา อาการมักจะเกิดขึ้นแบบค่อยเป็นค่อยไป จึงทำให้ความสามารถในการมองเห็นของผู้ป่วยลดลงอย่างช้าๆ

📌 โดยทั่วไปผู้ป่วยโรคจอประสาทตาเสื่อม จะมีสัญญาณเตือนดังต่อไปนี้

- มองเห็นไม่ค่อยชัด

- ภาพที่เห็นบิดเบี้ยว

- เห็นสีผิดเพี้ยน

- มีจุดดำกลางภาพ

- มองในที่สว่างไม่ชัด/แพ้แสง

เงื่อนไขการรับบริการ

1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำ E-Coupon มาแสดงเท่านั้น

2. ในกรณีที่ไม่นำ E-Coupon มาแสดง ท่านต้อง ชำระค่ามัดจำ ตามราคาชุดตรวจ

3. E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้

4. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้

5. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น

6. ชุดตรวจสุขภาพ ไม่รวม ค่าแพทย์ในกรณีเจาะจงแพทย์ผู้เชี่ยวชาญในการตรวจสุขภาพและตรวจทางรังสี

7. ชุดตรวจสุขภาพ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง

8. สอบถามรายละเอียดเพิ่มเติมหรือนัดหมายเพื่อรับบริการได้ที่ Contact Center โทร.1719

9. สามารถรับบริการที่ ศูนย์จักษุ โรงพยาบาลกรุงเทพ ชั้น 5 อาคารโรงพยาบาลกรุงเทพ ทุกวัน เวลา 8:00 - 16:00 น.

10.ราคานี้สำหรับคนไทยและ Expat เท่านั้น

วันหมดอายุ E-Coupon

1.E-Coupon มีอายุการใช้งานถึงวันที่ 31 ตุลาคม พ.ศ.2569

2.หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้

3.แพ็กเกจราคานี้สามารถสั่งซื้อได้ตั้งแต่วันที่ 25 กรกฎาคม - 31 ตุลาคม พ.ศ.2569 เท่านั้น

หมายเหตุ

1. ในกรณีที่ลูกค้าประสงค์จะยกเลิกคำสั่งซื้อด้วยเหตุผลที่ไม่เกี่ยวข้องกับโรงพยาบาลหรือเหตุผลทางการแพทย์ โรงพยาบาลขอสงวนสิทธิ์ในการคืนเงินหรือยกเลิกสินค้า

2. ยอดเงินที่ระบุในใบเสร็จเป็นยอดชำระจริง ไม่รวมส่วนลดจากโปรโมชั่นใด ๆ

3. หลังการสั่งซื้อ ลูกค้าจะได้รับใบเสร็จในรูปแบบออนไลน์เท่านั้น หากต้องการใบเสร็จตัวจริงในวันที่เข้ามารับบริการ สามารถแจ้งขอได้ที่เจ้าหน้าที่การเงิน

1.Service is reserved exclusively for customers who present the E-Coupon.

2.This E-Coupon cannot be exchanged for cash.

3.E-Coupon is only applied to specific items as mentioned and changes cannot be made.

4.These packages are for outpatients who are physically dependent.

5.These packages exclude doctor fees in case additional specialists are required.

6.These packages exclude treatment fees or medications prescribed by the doctors, if any.

7.Please make an appointment at least 3 days in advance.

8.For further information or to schedule an appointment, please contact the Contact Center at 1719.

9.Services are available at Bangkok Eye Center, 7nd floor, D Building, Bangkok Hospital, every day from 08:00 AM to 04:00 PM

10.This price is for Thai citizens and expatriates only.

Expiration Date of E-Coupon:

1. The E-Coupon is valid until October 31, 2026.

2. If the E-Coupon expires, the hospital reserves the right to deny service access, and the E-Coupon cannot be extended or refunded.

3. This health program is available for purchase only from July 25 to October 31, 2026.

Note:

1. In cases that customers request to cancel an order for reasons not related to the hospital or medical purposes, the hospital reserves the right not to issue a refund or cancel the product at its discretion.

2. The receipt amount displayed is the actual payment received, excluding any promotional discount.

3. After completing the purchase, customers will receive an online receipt only. If a physical receipt is required on the day of the visit, it can be requested from the cashier.
