เงื่อนไขการใช้บริการแพ็กเกจ สามารถซื้อแพ็กเกจได้ตั้งแต่วันนี้ – 30 มิถุนายน 2569 แพ็กเกจที่มีมากกว่า 1 ครั้ง สามารถใช้สิทธิ์ได้ถึงวันที่ 31 กรกฎาคม 2569 หากเกินระยะเวลาที่กำหนด โรงพยาบาลขอสงวนสิทธิ์ในการยกเลิกสิทธิ์การใช้แพ็กเกจดังกล่าว ราคาดังกล่าวรวมค่าเวชภัณฑ์ และค่าแพทย์ทำหัตถการเท่านั้น ไม่สามารถใช้ร่วมกับโปรโมชั่นหรือส่วนลดทางการตลาดอื่น ๆ ได้ โรงพยาบาลขอสงวนสิทธิ์ในการเปลี่ยนแปลงราคาโดยไม่ต้องแจ้งให้ทราบล่วงหน้า กรุณานัดหมายล่วงหน้าอย่างน้อย 3 วันก่อนเข้ารับบริการ เมื่อชำระเงินแล้ว ไม่สามารถเปลี่ยนแปลงรายการหรือขอคืนเงินได้ทุกกรณี รายการที่แพ็กเกจไม่ครอบคลุม ค่าตรวจประเมิน/ค่าปรึกษาแพทย์ และค่ายาทุกกรณี ค่าใช้จ่ายที่เกี่ยวข้องกับโรคประจำตัวของผู้ป่วย ค่าใช้จ่ายก่อนการทำหัตถการ ภาวะแทรกซ้อน และค่าใช้จ่ายอื่น ๆ นอกเหนือจากที่กำหนดในแพ็กเกจ ไม่รวมยากลับบ้าน เวชภัณฑ์เพิ่มเติม หรือวิตามิน (ถ้ามี) Terms & Conditions The package is available for purchase from now until 30 June 2026. Packages with multiple sessions must be utilized by 31 July 2026. The hospital reserves the right to invalidate any unused sessions after the specified expiry date. The stated price includes medical supplies and physician procedural fees only. This package cannot be combined with other promotions or marketing discounts. The hospital reserves the right to change prices without prior notice. Please make an appointment at least 3 days in advance. All purchases are non-refundable and cannot be changed under any circumstances. Exclusions Doctor consultation, evaluation fees, and medications in all cases. Expenses related to underlying medical conditions, pre-procedure costs, complications, and any additional expenses beyond the specified procedure. Take-home medications, additional medical supplies, or vitamins (if any) are not included.
